Utilizing Telemedicine and Mobile Health Platform in Facilitating Acute Heart Failure Treatment

Heart failure is one of the most important reasons for hospital admission and is associated with high mortality and morbidity.Optimal and aggressive titration of guideline-directed medical therapy is the cornerstone of heart failure management to reduce heart failure related hospitalization and improve patient survival.However, the percentage of patient receiving targeted dose of medical therapy in real-world setting is consistently low across different regions.Severe hospital bed and healthcare manpower shortage in our public healthcare system constrain the optimization of medical therapy for patients with heart failure.Continuous monitoring and automatic transmission of patient hemodynamic data using wireless and wearable devices might help to mitigate the issue of medication under-utilization.Access to these technologies is limited for many patients under the public healthcare system due to cost concerns.
A heart rate monitoring watch and a Bluetooth enabled electronic blood pressure devices will be provided to patients with heart failure upon discharge from the hospital for remote monitoring.A mobile platform consisting of a smart phone apps and a web-based platform will be developed.Data from the remote monitoring will be incorporated onto the mobile platform through which advices for medication titration and education on heart failure management will be provided to the patients.
This project will inform future heart failure management and help to formulate the optimal follow-up scheduling and medication titration regime for heart failure patients.Implementation of telemedicine in acute heart failure care will help to maximize healthcare resources utilization and facilitate fast adoption optimal medical therapy.Healthcare disparity will be eliminated with the promotion of mobile healthcare and personalized medicine.
