(a) Completed application form (MS Word), which could be downloaded from CEPU website;
(b) Declaration form endorsed by Head of relevant academic unit via the e-Declaration System (details available here); and
(c) VeriGuide scanning results; and
(d) CUHK research safety / ethics approval (if applicable) (in PDF).
An acknowledgement email would be issued to applicants upon receipt of application materials.
